Work & Access Survey Templates

Available from 5.5.0.0

Survey templates controlling safety and compliance requirements for site access and task execution. Attached to Sites, Buildings, Locations, Instruction Sets, Hazard Types, and Permit Classes to enforce mandatory survey completion at key workflow stages.

Enforces requirements at assignment, mobile check-in, completion, and history stages across Evolution, Evolution Go, Connect, and Contractor systems. Prevents assignment or completion until required surveys are answered.

Assignment ( MRI Evolution, Evolution Connect and Contractor Gateway)
- It is possible to configure the system so that an individual Resource cannot be assigned unless they have completed a valid response to the required survey
- It is possible to configure the system so that a Task cannot be assigned unless someone has completed a valid response to the required survey
Mobile and Contractor ( Evolution Go and Contractor Kiosk/Relay)
- It is possible to define the point in the Evolution Go Tasks activity that a resource would be expected to complete a survey for a Task
- It is possible to define the point in the Contractor Gateway and Contractor Kiosk that a user would be expected to complete a survey for a Task
Completion ( MRI Evolution and Evolution Connect)
- It is possible to configure the system so that a Task cannot be marked as Completed unless someone has submitted a valid response to the required survey
Send to History ( MRI Evolution and Evolution Connect)
- It is possible to configure the system so that a Task cannot be sent to History unless someone has submitted a valid response to the required survey

Access Surveys can be added to the following entities:

Work Surveys can be added to the following entities

Work Permit Classes
Hazard / Alert Types
Contracts
Instruction Sets

Along with Supply Chain Survey Templates they have replaced the Survey Templates functionality, added in the 5.3.0.0 release.

The Template must be saved before the survey designer can be opened to add questions to the survey and survey detail, such as entity assignments can be created.

Settings > Surveys > Work & Access Survey Templates

The link will open the Work & Access Survey Template grid. It is a grid view which can be searched, sorted and customised as normal.

To add a new Work & Access Survey Template click New , to edit an existing Work & Access Survey Template click Edit . A pop up window will appear which will allow the creation or amendment of a Work & Access Survey Template.

Description	The general description of the Survey Template
Survey Class	Read only text - Work & Access Requirement
Category	From the auto-complete select the Survey Category The field label is a hyper-link that will open the selected record's input panel when clicked
Survey Type	From the drop down select the Type ( MRI Evolution entity the survey will be assigned to): Building Hazard / Alert Type Instruction Location Site Work Permit Class The entity must have Work Surveys enabled in Table Catalog to appear in the drop down
Version	The version number of the Work & Access Survey Template
Est Time (HH:MM)	Available from 5.6.0.0 The estimated amount of time to complete the Survey
Report Template	From the drop down select the template to display the survey The field label is a hyper-link that will open the selected record's input panel when clicked The system standard template is SV - 001
Survey Response Validity Period
Submitted Responses	From the drop down select when the validity period of the survey response, the options are: Valid Until Expiry Date - once answered, the survey response will not be required again until after the expiry date No Automatic expiry - the survey is only required to be completed once and does not automatically expire Single Use Only - the survey will need to be answered each time
Expire Responses	If "Valid until Expiry Date" is selected in Submitted Responses an expiry must be set. The field is made up of three elements - a number field, a time period drop-down and a 'survey event' drop-down. The number field will determine a quantity of the chosen time periods that the Survey Response will be valid for after the chosen 'survey event'. Response expiry period examples: 6 - Months - After Issue 2 - Years - After Approval
Survey Response Behaviour for Mobile Devices and TeamWorks Contractor
Contractor	From the drop down select the expected Response Behaviour in Contractor, the options are: Not Applicable Required for each Task Required for each Work Permit - only available if the Survey Type is set to Work Permit Class
Prompt User	If the Response Behaviour in Contractor is set to 'Not Applicable' the drop down options are: Not Applicable If the Response Behaviour in Contractor is set to 'Required for each Task' the drop down options are: Gateway Task Details Before Starting Work During Work Before Finishing Work After Task Completion If the Response Behaviour in Contractor is set to 'Required for each Work Permit' the drop down options are: Gateway Task Details Before Starting Work During Work Before Finishing Work After Task Completion Gateway Permit Details
Evolution Go	From the drop down select the expected Response Behaviour in Evolution Go the drop down options are: Not Applicable Required for each Task Required for each Site Visit
Prompt User	If the Response Behaviour in Evolution Go is set to 'Not Applicable' the drop down options are: Not Applicable If the Response Behaviour in Evolution Go is set to 'Required for each Task' the drop down options are: On Acceptance Before Starting Work Risk Assessment Before Finishing Work If the Response Behaviour in Evolution Go is set to 'Required for each Site Visit' the drop down options are: On Acceptance On Arrival On Visit Completion
Survey Response Behaviour in MRI Evolution and Evolution Connect
MRI Evolution	From the drop down select the expected Response Behaviour in MRI Evolution Not Applicable Required for each Assigned Resource Required once per Task

Restrict to Role	Optional - Only users with the selected Role can complete a Survey Response for this Template. The field is read only if the Response Behaviour in MRI Evolution is set to 'Not Applicable' The field is enabled if the Response Behaviour in MRI Evolution is set to 'Required for each Assigned Resource' or 'Required once per Task'
Compulsory	If the Response Behaviour in MRI Evolution is set to 'Not Applicable' Not Applicable If the Response Behaviour in MRI Evolution is set to 'Required for each Assigned Resource' Not Applicable Not Compulsory Before Task Assignment Before Completion Before Sign Off If the Response Behaviour in MRI Evolution is set to 'Required once per Task' Not Applicable Not Compulsory Before Completion Before Sign Off

Complete the fields and click Save . Once the Work Survey Template header has been completed and saved, toolbar buttons and sub navigation options will appear.
